Plaintiff’s vehicle was struck from behind by a vehicle driven by defendant.

INJURIES/DAMAGES
Herniated disc at L4-L5; herniated disc at L5-S1. He underwent chiropractic treatment and physical therapy, and subsequently underwent a laminotomy and facetectomy. He continued to experience back pain, so an anterior and posterior fusion at L5-S1 was performed.
